Standardizing weight-based contrast volume per body region for contrast enhanced CT exams

Purpose: Standardizing the contrast volume administered during CT exams is crucial for maintaining consistent image quality and accurate diagnoses. Currently, there is a lack of agreement within the radiology community regarding the appropriate contrast volume for different body regions. This lack of consensus can lead to inconsistent image quality and suboptimal diagnostic accuracy.

Methods/Materials: We conducted a cross-sectional analysis of contrast-enhanced CT exams performed on 164 patients using a 128-slice GE machine over two months. The study included patients of varying weights who underwent CT scans of different body regions. We used Iohexol 350 mg I/mL as the contrast agent, with an average flow rate of 3.5 ml/sec (range:2.4-4). For adults, we used a volume estimate between 30 ml and 150 ml or a single injection dose of Omnipaque 350, calculated using 1.25 mL/kg of body weight (range:1- 1.5 for pediatric patients). Image quality analysis was recorded.

Results: We analyzed the contrast volumes administered for each body region and correlated them with patient weight. Our findings revealed a clear correlation between patient weight and the required contrast volume for different body regions in CT exams for weights ranging from 60 to 80 kg. Extremity angiographic studies tended to require larger contrast volumes to achieve optimal enhancement. However, contrast volume did not show significant variability for brain, neck, and soft tissue CT exams. Based on our analysis, we propose weight-based guidelines to standardize contrast volume adjustments for each body region.

Conclusions: Standardizing weight-based contrast volume per body region for contrast-enhanced CT exams is essential for ensuring consistent image quality and accurate diagnostic interpretation. The proposed guidelines will aid relevant healthcare professionals in determining the appropriate contrast volume based on patient weight, thus optimizing the diagnostic value of contrast-enhanced CT imaging.
